Create an exit survey
Only admins can create an exit survey to collect feedback from employees who are leaving the organization. After creating the survey, manually add employees from the exit survey report to send it to them. This helps identify the reasons behind attrition and areas for improvement.
1. Open the exit survey
Navigate to Apps > Butterfly EI > Exit Survey.
- If no exit survey exists, click + Setup your first Exit Survey. If an exit survey already exists, click Add Exit Survey at the top right.
⚠️ Warning: If you navigate away from the creation page before submitting the exit survey, your entries are not saved.
2. Set up the trigger and disclaimer
- (Optional) Under Disclaimer, enable Show disclaimer page to employees before they start the survey and enter the disclaimer text.
- Under Trigger question, select a custom trigger question from the Butterfly library, or click Add custom question to create one.
- (Optional) Enable Submit trigger answer only when all questions in the survey are answered.
- Under Hire Date Field, select the field that contains the hire date of the employee.
- Click Next.
3. Collect the reason for leaving
Select a reason-for-leaving from the Butterfly library or custom library.
- Select one of the following options:

- Butterfly Library: Select a question. By default, “What best describes your reason for leaving?” is selected.
- Custom Library: Select a question or click Add custom question to create one.

4. Set up questions for the survey
4.1. Add question categories
Question categories allow you to group questions and calculate a separate score for each group. Butterfly offers default question categories for the exit survey.
To create a new category:
- (Optional) Under Question Categories, click Add Category.
- Enter a category name.
- (Optional) Enter a code to display a custom icon for the category.
- Select a type of score: Average or Percent of favorable votes.
- (Optional) Enable Contributes to Overall Score to include the category score in the survey's overall score.
- Repeat these steps to add more categories. To remove a category, click the Delete
icon.
4.2. Add questions
Select questions from the Butterfly library or custom library as required.
Under Butterfly Library:
Butterfly offers default questions. By default, all questions are selected.
- Deselect questions as required.
- Under each question, open the drop-down to:

- Change the question category.
- Select the condition to specify when the question appears.
- Enable Optional if the question is not mandatory.
📒Note: If you do not deselect any questions, all questions from the Butterfly library are added to the exit survey, along with any custom questions you add.
Under custom library:
Add custom questions if the relevant questions are not available in the Butterfly library. For more information, refer to Add custom questions for a survey.
5. Manage notifications
- Enter a title and message.
- (Optional) Enable Send description in all supported languages to users whose language is unknown.
- (Optional) Enable Hide survey expiration time in invite.
- Under Send Reminders, select Enabled or Disabled.
- If “Send Reminders” is enabled, set the time and day to send reminder notifications.
- Click Next.
6. Manage languages
- Review the auto-generated translations under the General, Question Categories, and Question tabs.
- Edit the translations as required.
- Click Next.
7. Configure the general settings
- Enter a survey name. The name and description are visible only to you, not to employees.
- (Optional) Enter a survey description.
- (Optional) Enable Allow users to continue the survey where they left off.
- Click Submit to create the exit survey.
After creating an exit survey, add exiting employees to it to trigger the survey. For more information, refer to Add employees to the exit survey.
